Breast Forms
Triangular - Symmetrical
Trianglular shaped forms offer a comfortable bra cup fit. These symmetrical forms are designed to match most contemporary surgical techniques. Models 508, 528, 548, 520, 535.

Teardrop - Symmetrical
Suitable for women whose surgeries did not remove excess underarm or clavicle tissue, the teardrop form has an extension that can be angled slightly for a custom fit. Can be worn on either side. Models 509, 531, 537, 610.

Heart Shaped - Symmetrical
Fills the bra cup without unnecessary bulk when no axillary tissue has been removed in surgery. Model 518.

Asymmetrical - Shallow to full profile
The asymmetrical forms provide a range of cup projection and profile in the bra from shallow to full, depending on the extent of the surgery. Underarm extensions fill and blend where surgery has removed tissue. Models 503, 505, 506, 507, 510, 511.

Dark Tone Triangular Form - The triangular shape is very advanced, having a natural drape, lifelike feel and appearance. This form is ideal for darker complexioned women. It's unique translucent quality is specially designed to adjust to various skin tones when worn. |  |

Symmetrical triangular shape with rounded edges. A beautifully soft dual gel symmetrical shaped breast form. The soft front molds to fit many different bra cups and the firm back has a curved ripple molding for added comfort and support which allows for increased air circulation for a cooler fit. Tapered edges hug the chest wall for a smooth look. |  |

Teardrop Form - This form is available in the largest variety of sizes in the Naturalwear range, 17 in total. The A Supreme is an asymmetrical breat form with both upper chest and underarm extensions making it very suitable for use in more extensive surgeries. following codes to Medicare for reimbursement:
L8020 - Breast Prosthesis, Mastectomy Form (prosthesis to replace the breast after surgical removal).
L8030 - Breast Prosthesis, Silicone or Equal (prosthesis made of silicone gel to replace the breast after surgical removal).
